  1. Note: All employees that travel to a company and work at a customer’s plant are required to complete this document and return to the Customer Service Manager within four days of returning from a service call.

  2. Note: A copy of this form will be placed in the SO job folder by the Customer Service Manager. A copy of this form should also be filed on the network with the equipment manual by the Customer Service Manager.
